RTO Form 14: Register Showing the Enrolment of Trainee(s) in the Driving School Establishments

• Go to https://parivahan.gov.in/
• Click “Informational Services” on the menu tab
• Hover over “Downloadable Forms” in the drop-down menu
• Click “All Form” 
• Click on “Form 14” under the vehicle-related category

How to fill RTO Form 14?

Form 14 of the RTO must be filled out for each and every trainee that is enrolled in a particular driving school. It can be filled out using the following steps:

• In the first part fill in the year of the register.
• In the second part, you must fill in the enrollment number of the candidate.
• In the next part, fill in your name and attach your photograph.
• Next, fill in your father’s name, and permanent address in the following points. Female candidates who are married may also write the names of their husbands. Applicants who are currently residing at a temporary residence must specify along with the length of stay at the current place. 
• Fill in your date of birth in point 5.
• In point 6, fill in the class of the vehicle for which the candidate is training.
• In the next point fill in the date of the candidate’s enrollment.
• In the following points, you need to fill in the learner license’s number and its date of expiry, followed by the date of completion of the driving test competence.
• In point 11, fill in the assigned driving license number and date of issue, and the licensing authority which issued the license.
• In point 12, fill in the remarks, if any.
• You can finish the form by adding the signature of both the license holder and the instructor.
