RTO Form 5A: Driving Certificate Issued By Driving School Or Establishments

Driving is a necessary skill that many people must learn. Enrolling in a driving school, which may provide good training to make you a safe driver, is one way to learn properly. You can learn to drive for free from your parents, but it cannot be compared to receiving quality education from professional instructors.

Once you complete your training, the driving school or establishment will issue a driving certificate to you stating that you are eligible to obtain a driving license and drive within your country. This is done by using Form 5A.

How to Download RTO Form 5A?

Following are steps to download:

• Go to https://parivahan.gov.in/
• Click “Informational Services” on the menu tab
• Hover over “Downloadable Forms” in the drop-down menu
• Click “All Form” 
• Click on “Form 5A” under the vehicle-related category

How to fill RTO Form 5A?

Driving schools or establishments can fill form 5A using the steps given below:

• In the first blank you need to provide the name of the applicant followed by the name of their father or husband (in the case of married women) in the next blank.
• In the next blank, you need to fill in the complete address of the applicant as per their ID and Address Proof.
• This must be followed by filling their date of enrollment in the next blank.
• The next blank must be filled in correspondence with Form 14 (Register Maintained by Driving Schools or Establishments). This is to be filled with their registered serial number in your institution. 
• Further, you must fill in the class of the vehicle that they have trained to drive and the period of training (From and To Date).
• Finish the form by adding the signature, name, and designation of an authorized signatory.
• In the end, you need to provide the name and address of the driving school along with the license number and its date of issue.

• Form 14 (for correspondence on the registered serial number of the applicant)
• Learner’s License
