Parivahan Parent Consent Form: RTO Form for Learners License Under 18 Years Age

Even if you are under the age of 18, you can apply for a minor driving licence and this is where the parent consent form comes in. This form is for minors because when a minor applies for a diving/learning licence, they must download and sign the parent consent form as their authorization. How to fill RTO (Parivahan) Parent Consent Form?

Filling a parent consent form is fairly easy. Just use the following steps:

• In the first blank you need to fill in your child’s name.
• In the next blank, you need to provide your child’s age.
• The next thing you need to do is to make a mark on the yes checkbox which certifies that the person filling out the form has the right to submit a request in writing  to cancel the license in question and the registrar must comply with the request.
• The parent/guardian can finish the form by providing their signature in the provided space.
