  • In points 22 and 23, indicate whether you were excused from the medical test and preliminary examination under Central Motor Vehicles Rules, 1989, Rules 6 and 11(2).
  • In point 24, write about the fees spent for applying for a driver’s licence. Attach your signature or thumb impression, as well as the date of form submission, at the end of the personal information.
